Project Independence bridges the gap between the Program Participants’ discharge from the hospital and the return to their autonomy, independence, and quality of life at home. Patient Advocates work with a client in their home and support them to understand and follow discharge plans and medication orders, and coordinate and arrange for community services which may be helpful. Patient Advocates must have their own transportation, complete 20 hours of training, and meet every 3rd Tuesday each month from 10A-12noon for on-going training.
